A new scam has hit the Plateau area. It involves a form that has been sent to at least one doctor in Crossville requesting patient information. The document appears to be from a pharmacy and is titled “HIPAA Compliant Physician Authorization Form To Confirm An Active Patient”. It instructs the doctor to verify that the person listed on the form is still a current patient. Authorities are alerting the public and physicians that this is a scam.
